Please provide the following information:

Organization's name, address, phone number, fax and e-mail.
Name and title or person to be contacted

Brief statement of:

The history of organization
The purpose of the grant request
Your mission statement
How does your organization align to the Lattner Family Foundation Mission Statement

Supplemental information about the organization:

List of officers and directors and the year they joined the board.
Federal Tax Determination Letter for 501(c)(3) from the IRS with your number on it.
Your most recent 990 needs to be mailed to the office.
Your Expenditure Information (fill out the downloadable form below)

Important Note:

Effective July 29, 2026, we will be transitioning to a new software platform. On this date, at 7:00 A.M. EST, any prior information history you see on your end when “applying for a grant” will no longer be available to you.

We suggest that if you wish to save that information, please download your history to a storage device.

Additionally, we recommend those organizations interested in “applying for a grant” in the fall cycle, please do so on or after 7/29/26 to avoid any disruptions.
