• The history of organization

• The purpose of the grant request

• Your mission statement

• How does your organization align to the Lattner Family Foundation Mission Statement

Supplemental information about the organization:

• List of officers and directors and the year they joined the board.

• Federal Tax Determination Letter for 501(c)(3) from the IRS with your number on it.

• Your most recent 990 needs to be mailed to the office.
